Discretionary Fund Application Information Sheet
The attached Discretionary Fund Application is designed to assist us in determining our ability to provide financial assistance to you. The following information may help you understand how our financial assistance program works.
- The attached Discretionary Fund applicant must be completed and submitted to your counselor before financial assistance can be provided.
- Financial assistance will be provided to the families of patients that demonstrate a financial need that is a direct result of a patient’s illness (such as loss of income, incurred
travel expenses).
- The Christopher’s Cure Hardship Fund will only pay for expenses that have a direct impact on the patient’s quality of care. The types of expenses we may be able to assist you with include:
- Mortgage or Rent
- Transportation
- Utilities
- Child Care Facilities
We cannot assist you with:
Medical Expenses
- Medical Insurance Payments
- Credit Card Payments
- Cable TV
- Clothing
This assistance is available on an intermittent basis only, not on a regular basis.
Assistance is limited and determined on financial need.
